There is still the commercial problem here.  "Free" web space has
to be less attractive than explicitly paid for options, and the
ability to configure the server is one of the things that ISPs will
exclude from their "free" offerings.  Unfortunately, the result is
that people learn work arounds like the (HTML specification: should
not) use of meta refresh.

I assume you are talking about content management systems.  I think
part of the problem is the desire for branding and consistency, but
part of the problem is that HTML no longer meets its original
design aims of being authorable by the man in the street.
