The book is probably marked up with <dfn> tags, of which an index is 
generated. If the abbreviation is important, and appears in more places 
in the book, then the reader can consult the index.

Otherwise, they can still flip back a few pages to see where it is 
defined. Another option is that the author might want to mark up every 
first appearance of the abbreviation in the chapter (instead of every 
instance).

Simply put, in a real book (disregard XHTML for now), does it expand 
abbreviations on every occurance? No it doesnâ??t. So this is a non-issue 
for print material, or at least not one for which a practice hasnâ??t 
already been established.
