[XHTML2] "case" missing from element index and hyperlinking

Although switch is currently (see arguments for removal, elsewhere) in 
XHTML2 and switch requires case for it to make sense, case is not in
the index of elements.

Even where case is mentioned, close to switch, in the XFORMS Module,
it is not hyperlinked in the way that switch is.
